TRAINER REGISTRATION

1. To be a part of SukaSPORT official trainer, you are required to complete your information which is available in the registration form, start from your personal profile, training experience, and any supporting training certificates. We emphasize the need for the data that you fill in the form to be true and accurate, in order to provide accurate user review.
2. We suggest all trainers to provide SukaSPORT with 2 (two) active phone number, in the case an emergency.
3. SukaSPORT has the rights to review all trainers according to the SukaSPORT rating standard.

 
TRAINER REGISTRATION RULES

1. All trainers are required to subscribe to the SukaSPORT Newsletter.
2. SukaSPORT has the rights to reject any trainer registration for applicants with previous criminal or has done any conducts in the violation of law.
3. Trainer registration is only valid for one person that officially registered in SukaSPORT.
4. All trainers are to request permission for any replacement trainer if they were not able to reschedule training.
5. Replacement trainers must provide accurate and related information in order to be able to train SukaSPORT customers and protected by the SukaLIVE insurance program.

 
TRAINER PROFILE

1. For the security for trainers and users, we strongly recommend all trainers to fill in the trainers profile completely and accurately.
2. All trainers are strictly forbidden to falsify data regarding their identity and training qualification.
3. All trainers will be recorded in the SukaSPORT database to help users receive complete and comprehensive information about their trainer.
4. All charges resulted from the falsification of Trainers’ profile and inaccurate data given by non-compliant trainers are outside the scope of SukaLive and SukaSport responsibility.

 
TRAINER ETHICS

1. All trainers should not discriminate their users.
2. All trainers are expected to be professional and polite to SukaSPORT’ users.
3. All trainers are prohibited to offend their clients regarding issues of ethnicity, religion, race, and other identity groups.
4. All trainers are required to use appropriate clothing upon meeting their clients for the training session.
5. All trainers are required to ask permission upon physical contact with their clients upon training session. This is important to prevent clients from feeling uncomfortable and trainers being inaccurately accused of any type of harassment. Any type of harassment will result to being blacklisted from being a trainer of SukaSPORT.
6. All trainers are required to come on time based on training schedule and fulfill the training conditions promised by the trainer in question.
7. All trainers are responsible in learning about their clients’ previous injury history and train users in safely conduct.
8. SukaSPORT reserves the right to suspend trainers who violates the ethical rules that has been stated in the terms and condition.

 
TRAINER PAYMENT METHOD

1. Upon registering, trainers have agreed to the terms of payment that is applicable to SukaSPORT.
2. Trainers will receive 85% fee from total payment of the training session agreed.
3. Payment will be done once times a month, on the day 15 of every month.
4. Training session done on day 15 of the month or any time after will be paid on the next payment date.

USER REGISTRATION

1. We emphasize the need for the data that you fill in the form to be true and accurate, in order to provide accurate trainer review.
2. We suggest all users to provide SukaSPORT with 2 (two) active phone number, in the case an emergency.
3. Upon January 2017, SukaSPORT has decided to work together with JAGADIRI insurance from PT Central Asia Financial (CAF). In ordef to provide quality training for users, SukaSPORT requires all training session purchased after January 2017 to be insured in order to protect our users and trainers. Please refer to terms of condotion provided by JAGADIRI insurance from PT Central Asia Financial (CAF) below.

 
USER REGISTRATION RULES

1. Users are required to subscribe to the SukaSPORT Newsletter.
2. SukaSPORT has rights to suspend users if individual is found to violate the SukaSPORT terms and condition.
3. User’s registration is only valid for the person officially registered to SukaSPORT.
4. In the case that users are booking training session for family or friends, students must provide previous health records to SukaSPORT in order to allow trainer to provide safe training for the student.

 
USER PROFILE

1. User profile must be filled in completely and accurately.
2. SukaSPORT will not be responsible for injuries or unsatisfied training caused by falsify data given by the users (This include previous medical history, place, time, location of training and any others that may be crucial for training session).
3. All of the users' data will be recorded in SukaSPORT database to help trainers receive complete and accurate information to conduct their training session.

 
USER ETHICS

1. All users should not discriminate their trainers.
2. All users are expected to be professional and polite to SukaSPORT’ trainers.
3. All users are prohibited to offend their trainers regarding issues of ethnicity, religion, race, and other identity groups.
4. All users are required to use appropriate clothing upon meeting their trainers for the training session.
5. SukaSPORT recommend all users to voice their opinion if they are feeling uncomfortable with the trainer’s way of training.
6. SukaSPORT will not tolerate any type of harassment to our trainers and will report any cases directly to authoritative party.
7. All users are required to come on time based on training schedule. If users fail to inform SukaSport for any changes in training information, we will consider it as a legitimate training session.
8. All users are expected to have knowledge and understanding of all forms of risk associated with the use of service provided by SukaSPORT.
9. SukaSPORT has the right to suspend users who violates the ethical rules that has been stated in the terms and condition.

 
USER PAYMENT METHOD

1. Upon registering, users have agreed to the terms of payment that applicable to SukaSPORT.
2. All users are required to make a payment at least 3 (three) days before the exercise day.
3. Please send your proof of payment via email to cs@sukalive.com or WhatsApp to +62 813 8031 2471. (Subject: Proof of Payment [your name] [your exercise]).
4. Payment should be done in 24 hours after users confirm the booking. SukaSPORT has the right to cancel any booking if proof of payment is not been sent.
5. Users can cancel any booking through sending confirmation letter by email to cs@sukalive.com or WhatsApp to +62 813 8031 2471 at least 48 hours before training schedule. Please note the cancellation terms:

• If the cancellation is made in within 48 hours or more, SukaSPORT will refund 100% of your money in SukaSPORT credit.
• If the cancellation is made less than 48 hours, we will deduct 50% of your first session cost and refund the rest in SukaSPORT credit.
• If the cancellation is made on the day of training without any valid reason, then the training will be considered legitimate and no refund will be available to the user.

6. If the cancellation caused by force Majeure situation, then SukaSPORT will provide relief and allow users to reschedule training in the conditions that the user has filled out the reschedule form with evidence of the force Majeure situation. Here are some cases of force Majeure situations that we have recognized:

• Accidents
• Natural disaster
• Sudden illness
• Other cases not listed will be considered upon receiving the reschedule form.

7. Users are require to fill the refund form with accurate information before we approve the refund procedure.

 
USER INSURANCE

Your exercise in SukaSPORT is insured by JAGADIRI insurance from PT Central Asia Financial (CAF) with the following detail:

• The insurance is an Accident Insurance.
• This insurance only valid if you have an accident during your exercise.
• All of the accident that happened before / after the exercise session is not coverage by this insurance.

Insurace FAQ:

What is meant by death benefit due to accident?
In the case of the Insured person died by accident within insurance coverage period and policy still applied, then 100% insurance coverage will be paid then the insurance coverage ends.

What is meant by compensation for hospital treatment due to accident?
Cash benefit as a replacement of hospital treatment cost due to body injury by accident. Maximum benefit given is 10% from insurance coverage per event for maximum 30 days after accident. Compensation for hospital treatment due to accident will not decrease death benefit due to accident.

 
Meaning of terms

1. Treatment cost is all reasonable cost needed for treatment while having hospital treatment due to body injured or accident.
2. Body injured is the damage on body caused merely and directly by accident, can be medically proven in 90 (ninety) calendar days since accident causing Insured experience a covered event.
3. Medically needed is the usage of medical service, equipment and / or medical treatment / cure by doctor recommendation which are:
4. Consistent and reasonable based on diagnose and common western medical treatment for that particular condition.
5. Follow a valid standard medical treatment and have a medical benefit
6. Not looking for self advantage of Insured and / or Doctor
7. Treatment done in the safe place economically, efficient and not experimental.
8. Doctor is someone with doctoral certificate with western medical education background, registered and own permit letter from legal health authority. That Doctor must give diagnose as per doctoral ethical code with international validation. Not include in Doctor definition if the particular person is the Insured one or a person who owns family relationship with the Insured, unless with the previous written approval from the Insurer.
9. Accident is the event experienced by the Insured within this Policy period and can be medically proven, happens abruptly, unintended and unexpected.
10. Proper and reasonable is all cost reimbursed for the treatment, availability or service needed by medic to treat the Insured condition and not over cross the reasonable cost for similar treatment, availability or medical service in area of which those cost are reimbursed; not include cost that should not exist if there is no insurance. Proper and reasonable in this terms applied to valid medical standard service with regulation of Republic Indonesia Health Ministry.
11. Treatment is a medical procedure done by Doctor in the purpose to cure and restore body function merely and directly caused by or as a cause of illness and/or body injury due to accident happen on the Insured during insurance period.
12. Hospital treatment is the proper and reasonable outpatient and inpatient treatment, medically needed by the Insured on recommendation and under surveillance also routine  treatment by Doctor who is merely and directly caused by or a result from the insured event.
13. Insured event is a body injury due to accident happens on the Insured during insurance period as per this policy terms.
14. Inpatient is doctoral treatment provided for the patient in form of temporary treatment in hospital within specific time period.
15. Outpatient is doctoral service provided for the patient not in the form of inpatient.
16. Insured is individual under 71 year old on the insurance start date.

 
Exception
Insurance coverage based on this policy is not valid and benefit will not be paid by the insurer if there is an insured event, directly or indirectly caused by things below:

• War (with or without war declaration), foreign invasion into a country, conflict of one country to another country, terrorism, civil war, rebellion, revolution, chaos, military activity;
• Insured involvement in a military duty of an Armed Forces or international agencies;
• Intentional or carelessness from people who insure, the insured or people who deserve to receive benefit from this policy;
• Suicide attempt or any other similar attempt whether in conscious or unconscious, arrangement to do suicide attempt or other effort to hurt themselves;
• Insured do or involve in action against country law and/or rule applied in a country where that kind of act done by the Insured;
• Use drugs, narkotics and/or other addictive subtances, liquor and/or other intoxicating drinks;
• Pregnancy, birth or miscarriage and complication happen as a cause;
• Nuclear reaction, radiation or addictive substances contamination
• Complain or brain disease and mental disorder;
